Responsibilities

• Provide social work services to high-risk HIV adolescents and their families in an integrated medical/mental health setting as part of a team.
• Deliver short-term and ongoing individual, family, and group counseling, case management, crisis intervention and health education.
• Develop strategies to recruit and retain at-risk adolescents in care and coordinate care with medical providers.
