Job summary

Piedmont Augusta Hospital in Augusta, GA is seeking a Social Worker to promote patient and family wellness and coordinate transitions of care for patients with complex psychosocial needs in Inpatient and ED settings.

Requires a Master’s Degree in Social Work and 1 year of acute/post-acute experience. LMSW, ACM or CCM are preferred credentials, with a full-time on-site role supporting hospital teams and community resources.
