Job summary

Piedmont Atlanta Hospital is seeking a social work professional to promote patient and family wellness and coordinate transitions in inpatient and ED settings. You will develop comprehensive care plans for patients with complex psychosocial needs and collaborate with physicians, families and external agencies to improve outcomes.

Required is a Master’s Degree in Social Work with at least 1 year of relevant acute/post-acute experience; LMSW/ACM/CCM are preferred credentials.

Qualifications

• Master’s Degree in Social Work from an accredited social work program.
• 1 year of experience in an acute or post-acute setting.
• LMSW, ACM, and CCM are preferred credentials.
