Job summary

Piedmont Mountainside Hospital in Georgia seeks a Social Worker with a Master’s in Social Work to join our inpatient and ED teams. You’ll develop comprehensive transitional care plans for patients with complex needs and collaborate with physicians, families, and external agencies.

No experience required; ACM/CCM/LMSW certifications are preferred to support clinical excellence and ongoing professional growth within a compassionate care environment.

Qualifications

• Master's Degree in Social Work from an accredited social work program is Required
• No experience required; CCM/ACM/LMSW preferred or equivalent certifications

Responsibilities

• Promote patient and family wellness and improve care outcomes through coordinated transition planning for patients with complex psychosocial needs.
• Collaborate with physicians, other service teams, patients, families, and external agencies to ensure effective transitions and resource access.

Responsibilities

This position strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Works to promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families.
