Job summary

Piedmont is seeking a dedicated Social Worker to support patients in Inpatient and ED settings, promoting wellness, safer transitions, and better outcomes for those with complex psychosocial needs.

You will collaborate with care teams, physicians, patients and families to connect hospital and community resources, develop comprehensive transitional care plans, and advocate for services that improve access and equity.

Qualifications

• Master's Degree in Social Work from an accredited program.
• 1 year of experience in an acute or post-acute setting.
• LMSW, ACM, CCM licenses preferred but not required.

Responsibilities

• Promote patient and family wellness and improve care outcomes.
• Develop and manage comprehensive transitional care plans for patients with complex psychosocial needs.
