Job summary

Piedmont Augusta Hospital is seeking a Master’s level Social Worker to promote patient and family wellness and coordinate safe transitions from inpatient and ED settings. You will develop comprehensive care plans for patients with complex psychosocial needs and collaborate with a multidisciplinary team, external agencies and families to improve outcomes.

Required is a MSW and at least 1 year of experience in acute or post-acute care; LMSW/ACM/CCM are preferred.

Qualifications

• Master’s Degree in Social Work from an accredited social work program required.
• 1 year of experience in an acute or post-acute setting required.

Responsibilities

• Promote patient and family wellness and improve care outcomes.
• Develop and support transitional care plans for patients with complex and psychosocial needs in Inpatient and ED environments.
