Job summary

Agape Care Group is seeking a hospice medical social worker to provide psychosocial evaluations and ongoing counseling to patients and families during end-of-life care in Woodward, OK.

This part-time role emphasizes compassionate support, adherence to the care plan, and collaboration with a supportive leadership team to ensure meaningful experiences for patients and families.

Qualifications

• MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Current state license as a social worker
• 2+ years of clinical work experience, preferably in healthcare or hospice
• Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.

Responsibilities

• Perform psychosocial evaluations and ongoing counseling for patients and families during end-of-life care.
• Provide emotional support to patients and families as needed, following the plan of care.
• Collaborate with leadership and care teams to ensure compassionate, patient-centered care.
