Job summary

Agape Care Group is seeking a compassionate Social Worker to join its hospice team in South Carolina. You will perform psychosocial evaluations, provide ongoing counseling to patients and families, and work in alignment with the plan of care to ease end-of-life journeys.

Qualifications include MSW from CSWE-accredited program, current state licensure, and at least 2 years of clinical hospice experience. Reliable transportation and physical ability to lift up to 25 lbs.

• MS degree in social work from an accredited school approved by the CSWE.

Responsibilities

• Conduct psychosocial evaluations for patients and families.
• Provide ongoing counseling and emotional support per the plan of care during the end-of-life journey.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
