Job summary

Agape Care Group in Oklahoma is seeking a part-time Social Worker (24 hours/week) to provide psychosocial evaluations, counseling, and emotional support to patients and families at end-of-life.

You will work within the plan of care, collaborate with the care team, and benefit from leadership support, professional development, and opportunities for advancement.

Qualifications

• MSW degree from an CSWE-accredited program.
• Current state social worker license.
• 2+ years of clinical social work experience in healthcare or hospice.
• Reliable transportation and ability to lift 25lbs.

Responsibilities

• Perform psychosocial evaluations for patients and families.
• Provide ongoing counseling and emotional support during end-of-life care.
• Coordinate with the plan of care and care team to address psychosocial needs.

As a hospice medical social worker, you will be responsible for psychosocial evaluations, and ongoing counse of patients and families during their end-of-life journey. Working in accordance with the plan of care, you will provide emotional support to patients and families when it's needed most.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
