Job summary

Agape Care Group in Elk City, OK is seeking a hospice medical social worker to perform psychosocial evaluations and provide ongoing counseling for patients and families during the end-of-life journey. You will work in accordance with the plan of care and offer emotional support when it is needed most.

Qualifications

• MS degree in social work from an accredited school of social work approved by the CSWE
• Current state license as a social worker
• 2+ years of clinical work experience, preferably in healthcare or hospice
• Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.

Responsibilities

• Perform psychosocial evaluations and counseling for patients and families.
• Provide emotional support throughout the end-of-life journey.
• Follow the plan of care and coordinate with care teams.

Qualifications

• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
