Agape Care Group is seeking a hospice medical social worker in South Carolina to provide psychosocial evaluations and ongoing support for patients and their families during end-of-life care.

The role includes emotional support, coordination with the care plan, and collaboration with a compassionate leadership team. A MSW with state licensure and 2+ years of clinical hospice experience is required.

Qualifications

• MS degree in social work from an accredited school (CSWE).
• Current state license as a social worker.
• 2+ years of clinical work experience, preferably healthcare or hospice.
• Reliable transportation; ability to sit, stand, bend, move intermittently and lift at least 25 lbs.

Responsibilities

• Conduct psychosocial evaluations and provide ongoing counseling to patients and families.
• Offer emotional support to patients and families during end-of-life care.
• Collaborate with the plan of care and interdisciplinary team to ensure compassionate, patient-centered care.

• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
