Job summary

Community LIFE is seeking a Social Worker to maintain a caseload of participants over age 55, perform psycho-social assessments and interventions, and act as the primary contact for utilization case management of acute inpatient admissions and transfers. The position is Monday through Friday, 7:30am to 4:00pm, with no nights or weekends.

Location: Lower Burrell, PA. Requires MSW from an accredited school and at least one year of geriatric experience; PSLF eligibility and a comprehensive benefits

Qualifications

• MSW from an accredited school of social work.
• Minimum of one year experience with frail/elderly population.
• Experience in geriatric social work preferred in hospital, long-term care, or community setting.

Responsibilities

Job description

Position Summary:

Our Social Workers maintain a caseload of Community LIFE participants (adults over age 55) and provide them and their families with psycho-social assessments and interventions related to the maintenance of the participants in the community. As the primary contact, the Social Worker performs utilization case management for all acute inpatient admissions and transfers. This position is Monday-Friday 7:30am-4:00pm with no nights or weekends!

Required Education:

Master's Degree in Social Work (MSW) from an accredited school of social work.

Required Experience:

Must have a minimum of one year experience working with the frail or elderly population. Geriatric social work experience preferred in a hospital, long term care or community setting.

Required Skills:

• Good verbal and written communication skills.
• Ability to work with people from various socio-cultural backgrounds in an effective manner.
• Ability to accept and utilize supervision.
• Ability to work independently and be self-directed.
• Good problem-solving skills.
• Must have a compassionate and empathetic attitude for the frail older adults.
