Our Social Workers provide complex care management in the primary care setting. Through sensitivity to social drivers of health, understanding of the role behavioral health challenges often play in a patient\'s overall health, and deep knowledge of socioeconomic safety net systems and resources, the Social Worker helps patients overcome barriers and make meaningful progress toward health-related goals. Summary:

Required Minimum Knowledge, Skills, and Abilities (KSAs)

• Education: Bachelor\'s Degree in Social Work from an institution accredited by the Council on Social Work Education required.
• License/Certifications: Active license by the Maine State Board of Social Work required.
• Experience: Experience providing therapeutic services to adult and pediatric patients with chronic medical issues required. Experience in client advocacy and case management required.

The pay range for this position is $61,256.00to $82,867.20per year.

Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
