Job summary

MaineHealth's Franklin Hospital is seeking a Social Worker for MHMG to provide professional social work services that support the Population Health program and deliver risk-based population management to patients throughout their care.

Required qualifications include a Bachelor's Degree in Social Work from CSWE-accredited program, active Maine State Board of Social Work license, and experience with therapeutic services for adults and children with chronic conditions, along with advocacy and case

Summary:
The Social Worker - MHMG role provides professional social work services that support the goal of the Population Health program to deliver risk-based population management services to patients at all times during their tenure of care.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Bachelor's Degree in Social Work from an institution accredited by the Council on Social Work Education required.
  2. License/Certifications: Active license by the Maine State Board of Social Work required.
  3. Experience: Experience providing therapeutic services to adult and pediatric patients with chronic medical issues required. Experience in client advocacy and case management required.

The pay range for this position is $61,256.00to $82,867.20per year.

Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).

In addition to base pay, MaineHealth offerscomprehensivebenefits, recognition programs, career growth opportunities, and,where applicable,shiftdifferentials or other incentives.
