Job summary

Andwell Health Partners in Scarborough, ME is seeking a Per Diem LCSW to provide hospice and home health social work services. The role focuses on assessing patient needs, coordinating care, and supporting families within community settings.

The MSW/LCSW-credentialed social worker will work to enhance quality of life, manage care plans, and educate staff, while staying compliant with licensure requirements and CPR certification.

Qualifications

• Master’s degree from an accredited school of social work.
• Maine Board of Social Work Licensure—MSW or LCSW required.
• Current CPR certification.
• One+ year of experience as a master’s level social worker in a health care setting.

Responsibilities

• Assess and identify the client’s holistic health care needs/concerns as part of a family and community.
• Assess social/emotional factors related to the patient’s illness, need for care, response to treatment, and adjustment to care.

What You’ll Do

• Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.
• Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.
• Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.
• Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
• Serve as resource for staff education & training, participate in case conferences as needed.
• Per Diem LCSW to support Palliative Care, Hospice, and Home Health

What You’ll Bring

• Master’s degree from a school of social work accredited by the council on social work education.
• Maine Board of Social Work Licensure- MSW or LCSW required
• Current CPR card required.
• One or more years of experience as a master’s level social worker in a health care setting.
• A valid Drivers License and reliable car with insurance.
