Job summary

Andwell Health Partners seeks a Medical Social Worker (MSW) to support hospice and home health patients in Lewiston, Maine. The role focuses on assessing holistic needs, coordinating care, and guiding families through illness and treatment decisions.

The MSW will develop and implement a patient-centered plan under physician orders, collaborate with the care team, and participate in case conferences. A valid MSW/LCSW and CPR are required, with at least one year of relevant health-care experience.

Qualifications

• Master’s degree in social work from a CSWE-accredited program.
• Maine Board of Social Work Licensure – MSW or LCSW.
• Current CPR certification.
• One+ year of experience as a master’s level social worker in health care.
• Valid driver’s license and reliable car with insurance.

Responsibilities

• Assess holistic health care needs in collaboration with client/family.
• Evaluate social/emotional factors related to illness and care needs.
• Develop a care plan with physician orders and measurable outcomes.
• Implement care activities toward independent living for patient/family.
• Serve as staff education resource and participate in case conferences.
