Lighthouse Care Center of Augusta, a 84-bed facility in Augusta, GA, is seeking a Social Worker (MA/LMSW) to join the Lighthouse Clinical Services Team. The role emphasizes patient-centered assessment and coordinating treatment for individuals with mental illness, behavioral issues, or substance concerns.

You will conduct psychosocial evaluations, coordinate discharge planning through medical staff, and work with families, guardians, and outpatient aftercare providers to ensure high-quality,

Qualifications

• Master's Degree in social work required.
• Georgia licensure required. LCSW, LMSW
• 2 to 5 years’ experience in mental health required.

• Makes referrals to community resources as appropriate.
• Coordinates discharge planning through the medical staff.
• Communicates with outpatient referral sources, families/guardians, and outpatient aftercare providers.
• Critical member of treatment team, interdisciplinary strategies and collaboration
• Completes psychosocial evaluations on assigned clients in a timely fashion.
• Conducts required therapy sessions with individuals, families and groups.

The Social Worker (MA/LMSW) opportunity is a key member of the Lighthouse Clinical Services Team who will integrate and coordinate a patient centric therapeutic strategy with a keen focus on patient care; ensuring that delivery of high-quality and cost-effective treatment is consistent with the mission, vision, and values of Universal Health Services and in accordance with government regulation, licensing and accreditation requirements. The Social Worker is responsible for comprehensive assessment of persons whose lives are disrupted or complicated by mental illness, behavioral disturbance, substance issues or inability to function or maintain in the community.
