Job summary

Centerpoint Health-Paris, part of Lifepoint Health, is seeking a Social Worker for a full-time role with 8:00 a.m. to 4:30 p.m. Monday through Friday and weekend rotations.

The position focuses on patient- and family-centered care, discharge planning, and coordinated services within a compassionate team. The ideal candidate holds a Bachelor’s in Social Work (required) and may have a Master’s preferred. Licensure within Kentucky and Handle With Care certification are requested within specified

• Bachelor's in social work is required.
• Masters degree is preferred.

Responsibilities

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress.
• Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes. Identifies resources and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.

How you'll contribute

A social worker who excels in this role:

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.

• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.

What we're looking for

Applicants should have a current bachelor's in social work (required). Additional requirements include:

• Education: Bachelors required; Masters preferred
• Licenses:Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire
• Certifications:Handle With Care within 30 days of hire.
