Job summary

Centerpoint Health-Paris, part of Lifepoint Health, is seeking a dedicated social worker to support patients and families, coordinate services, and contribute to discharge planning.

The role emphasizes collaboration with the healthcare team, case management, and providing individual, family or group therapy. Bachelors in social work and state licenses are preferred.

Qualifications

• Applicants should have a current bachelor's in social work (required).
• Licenses: Board Certified Social Worker or related field in Kentucky within 6 months of hire.
• Certifications: Handle With Care within 30 days of hire.

Responsibilities

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress.
• Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes; identifies resources and provides referrals.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.

Schedule: Full-Time | Monday - Friday w/ weekend Rotations | 8a-4:30p |

How You'll Contribute

A social worker who excels in this role:

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress.
• Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.

What We're Looking For

Applicants should have a current bachelor's in social work (required). Additional requirements include:

• Education: Bachelors required; Masters preferred
• Licenses: Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire
• Certifications: Handle With Care within 30 days of hire.
