Job summary

Centerpoint Health-Paris, part of Lifepoint Health, is seeking a Social Worker to join our team in Paris, KY. This full-time role supports patients and families, coordinates services, and contributes to discharge planning to improve community health outcomes.

You will provide individual, family and group therapy, participate in case reviews, and supervise staff and students as needed, while working within a compassionate care environment.

Qualifications

• Bachelor’s degree in social work required.
• Master’s degree preferred.
• Licensed or board-certified social worker in KY within 6 months of hire.

Responsibilities

• Interviews and assesses patients and families, caregivers or legal representatives.
• Coordinates needed internal and external services and interventions.
• Participates in case reviews to evaluate progress and management.
• Promotes treatment plan adherence through collaboration with the healthcare team.
• Assists with discharge planning and connects patients to resources.
• Supervises and/or trains new staff, students and interns.
• Provides individual, family and group therapy.
