Job summary

Valley View Care Center in Grand Rapids, MI is seeking a Social Worker to provide social services to residents and their families, coordinating care planning, referrals, and discharge planning. The role supports residents dealing with illness and placement, with weekend duties as part of the management team.

Requirements include a Bachelor's in Social Work, a Limited License in Social Work (LSW/LBSW) with LBSW preferred, and at least 1 year of supervised social work in healthcare; long-term care

Qualifications

• Bachelor’s Degree in Social Work is required or strongly preferred.
• Minimum of 1 year of supervised social work experience in a health care setting.
• Long-Term care experience is preferred.
• Excellent customer service skills with a commitment to quality care.
• Team player, dedicated to supporting colleagues and patient care.

Responsibilities

• Identify the medically-related social service needs of residents and assist with ensuring those needs are met by the appropriate discipline.
• Maintain contact with family (with resident’s permission) to report changes in behavior and care planning.
• Assist with financial and legal matters such as guardianship petitions and funeral arrangements.
• Coordinate discharge planning services and arrange home care or transfers as needed.
• Support residents’ individual needs and preferences through assessment and care planning.

How you will help people every day:

• Identify the medically-related social service needs of residents and assist with ensuring those needs are met by the appropriate discipline.
• Maintaining contact with family (with resident’s permission) to report on changes in behavior, psychosocial functioning, discharge planning, and encouragement to participate in care planning.
• Assisting residents/responsible parties with financial and legal matters such as drafting petitions for guardianship/conservatorship, assisting with pre-paid funeral arrangements, etc.
• Coordinating discharge planning services, i.e., helping to place a resident on a waiting list for community congregate living, arranging intake for home care services for residents returning home, assisting with transfer arrangements to other facilities.
• Through the assessment and care planning process, identifying and seeking ways to support residents’ individual needs and preferences, customary routines, concerns and choices.

What you will need to become a part of our EPIC team:

• Bachelor’s Degree in Social Work, or in related human services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling, and Psychology; must have a minimum of a Limited License in Social Work; LBSW preferred.
• 1+ years of supervised social work experience in a health care setting working directly with individuals.
• Long-Term care experience is preferred.
