Job summary

Evergreen is seeking a Social Services Director (LSW required) in Battle Creek, MI to lead the social services department and advocate for resident rights. You will oversee care management, identify residents' social needs, and coordinate psychiatric services to enhance overall well-being.

The role emphasizes patient-centered care, staff training, and collaboration with leadership to maintain exceptional service quality and dignity for residents and their families.

Qualifications

• Bachelor’s Degree in Social Work or related field.
• LSW licensure required (LBSW preferred).
• 3+ years of related experience in a health care facility, long term care preferred.
• Knowledge of gerontology is highly desired.
• Excellent customer service skills with a commitment to compassionate care.

Responsibilities

• Oversee and direct resident care management and ensure social services provisions are carried out.
• Identify medically-related social services needs of residents and ensure that these needs are met.
• Assist in meeting residents' psychological needs, including developing and managing services to improve social functioning and dignity.
• Coordinate psychiatric services and monitor psychoactive medication usage.
• Promote hospitality and excellent customer service in daily interactions with guests, families, and visitors.
• Develop and deliver clinical orientation and training for nursing staff, including policy training and performance assessment.

The Social Services Director oversees and directs the daily operations of the social services department. This position is responsible for leading efforts to provide social services to residents and their families to enable them to deal with the impact of illness and extend placement. The Social Workerparticipates in resident care management and plays a key role in advocating and promoting resident rights.

How you will help people every day:

• Oversee and direct resident care management and ensure all established social services provisions are carried out.
• Identify medically-related social services needs of residents and ensure that these needs are met.
• Assist in meeting the psychological needs of each resident, including developing and managing services to improve social functioning, reduce psychosocial problems, enhance positive self-image, and preserve dignity.
• Coordinate psychiatric services and monitor psychoactive medication usage.

What you will need to be part of ourEPICteam:

• Bachelor’s Degree in Social Work, or in related human services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling, and Psychology; must have a minimum of a Limited License in Social Work; LBSW preferred.
• 3+ years of related experience in a health care facility; long term care experience
• highly desired; working knowledge of gerontology.
