Job summary

OakPointe Senior Care & Rehab Center in Detroit, MI seeks a Social Worker to provide social services to residents and families, support care planning, and coordinate community resources. This full-time role emphasizes compassionate, collaborative care within an award-winning team.

You will engage in discharge planning, family communication, and assistance with guardianship or financial matters, delivering excellent, patient-focused service with a supportive team.

Qualifications

• Degree in Social Work or related field preferred but not required.
• Licensed in Social Work preferred but not required.
• 1-2 Years of supervised social work experience in a health care setting.
• Excellent customer service skills with a commitment to delivering quality care.
• You are a team player, dedicated to supporting your team.

Responsibilities

• Identify the medically-related social service needs of residents and assist with ensuring those needs are met by the appropriate discipline.
• Maintain contact with family (with resident’s permission) to report on changes in behavior and discharge planning.
• Assist residents with financial and legal matters such as drafting petitions for guardianship or conservatorship.
• Coordinate discharge planning services and arrange intake for home care services.
• Identify and support residents’ individual needs, routines, and preferences during assessment and care planning.

Education

Degree in Social Work or related field

Licensed in Social Work (preferred)

1-2 Years of supervised social work experience

Excellent customer service skills

What you will need to become a part of our team:

• Degree in Social Work, or in related human services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling, and Psychology is preferred but not required.
• Licensed in Social Work preferred but not required.
• 1-2 Years of Supervised social work experience in a health care setting working directly with individuals.

How you will help people every day:

• Identify the medically-related social service needs of residents and assist with ensuring those needs are met by the appropriate discipline.
• Maintaining contact with family (with resident’s permission) to report on changes in behavior, psychosocial functioning, discharge planning, and encouragement to participate in care planning.
• Assisting residents/responsible parties with financial and legal matters such as drafting petitions for guardianship/conservatorship, assisting with pre-paid funeral arrangements, etc.
• Coordinating discharge planning services, i.e., helping to place a resident on a waiting list for community congregate living, arranging intake for home care services for residents returning home, assisting with transfer arrangements to other facilities.
• Through the assessment and care planning process, identifying and seeking ways to support residents’ individual needs and preferences, customary routines, concerns, and choices.
