Valley View Care Center in Grand Rapids, MI seeks a Social Worker to provide resident and family social services, participate in care management, and coordinate referrals. Weekend duties with other managers are included, with plenty of community involvement and support from an award-winning team.

The role requires a Bachelor’s in Social Work, LSW or Limited License, and at least 1 year of supervised social work in a health care setting. Excellent customer service and teamwork are essential.

Qualifications

• Bachelor’s Degree in Social Work (required).
• 1+ years supervised social work in health care setting.
• Long-Term care experience preferred.
• Excellent customer service and teamwork.

Responsibilities

• Identify medically-related social service needs and coordinate with suitable disciplines.
• Maintain contact with family to report changes and support care planning.
• Assist with financial and legal matters for residents; support guardianship/pre-paid arrangements.
