Job summary

WellBridge of Romeo is seeking a dedicated Social Worker to provide social services to residents and families, ensuring needs are met through programming and referrals. The role involves weekend duties with other managers and supports care planning and discharge coordination.

Requirements include a BA in Social Work or related field, an LSW license, and 1+ year in health care social work; long-term care experience is preferred. Competitive wages and extensive benefits are offered.

Qualifications

Responsibilities

• Identify medically-related social service needs of residents and coordinate with the appropriate discipline.
• Maintain contact with family (with resident’s permission) to report changes and participate in care planning.
• Assist residents/parties with financial and legal matters such as guardianship petitions and funeral arrangements.
• Coordinate discharge planning and arrange home care services or transfers as needed.
• Identify and support residents’ individual needs and preferences during assessment and care planning.

The Social Worker provides social services to residents and their families to assist them in dealing with the impact of illness and extended care placement. This position participates in resident care management and ensures that needs are met through Social Service programming and appropriate referrals to outside agencies and community resources. This is position will participate in weekend duties with other managers.

How You Will Help People Every Day:

• Coordinating discharge planning services, i.e., helping to place a resident on a waiting list for community congregate living, arranging intake for home care services for residents returning home, assisting with transfer arrangements to other facilities.
• Through the assessment and care planning process, identifying and seeking ways to support residents’ individual needs and preferences, customary routines, concerns and choices.

What you will need to become a part of our EPIC team:

• Bachelor’s Degree in Social Work, or in related human services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling, and Psychology; must have a minimum of a Limited License in Social Work; LBSW preferred.
• 1+ years of supervised social work experience in a health care setting working directly with individuals.
• Long-Term care experience is preferred.
