Job summary

Care Resources is seeking a Social Worker (MSW) for a full-time role at Care Resources PACE in Kentwood, MI. The position involves conducting psychosocial assessments, creating and updating care plans, and providing counseling and advocacy for older adults and their families.

The role requires a Master’s in Social Work and current Michigan LMSW license, with on-site duties Monday to Friday and a rotating weekend on-call schedule. Prior experience with the frail elderly is preferred.

Qualifications

• Requires Master’s Degree in Social Work and current state of Michigan LMSW license (LLMSW acceptable with intent to obtain full licensure).
• Prefer at least 2 years of relevant experience.
• Minimum of 1 year experience with frail or elderly population.
• Ability to manage multiple tasks while staying organized and professional.
• Work well autonomously as well as within a team setting.

Responsibilities

• Conduct initial psychosocial assessments for program participants and consult with families.
• Develop clear written social service care plans and implement them.
• Review and revise care plans as needed.
• Assist families with problem solving around caregiving and facilitate family meetings as needed.
• Provide crisis intervention and advocacy as needed.
• Provide supportive counseling to participants and families.
• Schedule: Mon–Fri 8:00am–4:30pm with weekend on-call rotation every 2 months.

What You’ll Do, includes but is not limited to:

What We’re Looking For
