Qualifications

• Master’s degree in social work from a CSWE-accredited program is required.
• Indiana Social Work License or eligibility for licensure is required.
• Clinical social work experience in a health care setting of at least 1 year is required.
• Knowledge of growth and development across the lifespan is required.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient and family needs and develop appropriate care plans.
• Provide counseling, crisis intervention, emotional support, and resource guidance to patients and families.
• Coordinate discharge planning and transitions of care, including referrals to community resources, extended care facilities, hospice, and support services.
• Collaborate with physicians, nurses, case managers, and other interdisciplinary team members to ensure quality patient care.
• Advocate for patients and families by connecting them with financial, social, behavioral health, and community resources.
• Educate patients and families on psychosocial issues, treatment plans, available services, and healthcare resources.
• Assess and respond to concerns related to abuse, neglect, safety, mental health, substance use, and other psychosocial risks in accordance with regulatory requirements.
• Facilitate support groups, family meetings, and care conferences as appropriate.
• Maintain timely, accurate documentation in the electronic medical record.
• Participate in multidisciplinary rounds, professional development activities, and departmental initiatives to support quality patient outcomes.
