Responsibilities

• Completes a thorough psychosocial assessment for new patients.
• Uses critical thinking to identify and screen high risk social cases.
• Provides pre-treatment planning, psychosocial counseling/interventions, health education, financial counseling, referrals, and discharge/transition planning.
• Collaborates with team, other departments, case management, and social service providers to develop a care plan.
• Provides resources and education to patients and families on financial assistance, transportation, scheduling, and community resources.
• Supports department initiatives (attends case conferences, participates in quality improvement).
• May perform other duties as assigned.
