Qualifications

• 0 to 2 years of relevant experience required.

Responsibilities

• Completes thorough psychosocial assessments for new patients.
• Identifies high-risk social cases through critical thinking and screening.
• Provides pre-treatment planning, psychosocial counseling, health education, financial counseling, referrals, and discharge planning.
• Collaborates with the care team to develop patient care plans.
• Provides educational materials and resources to patients and families for financial assistance, transportation, scheduling, and community resources.
• Supports department quality improvement initiatives and may perform other duties as assigned.
