Health & Human Services Comm in Vernon State Hospital seeks a Social Worker II to coordinate care, assess needs, and advocate for client rights within a forensic setting. You will work with a team to ensure person-centered planning and ongoing support.

As part of the Social Work team, you will participate in admissions, transfers, transitions, and discharge planning, while ensuring compliance with state policies and professional ethics. This role emphasizes collaboration and advocacy.

Qualifications

• Knowledge of individuals with intellectual and developmental disabilities, human growth and development, behavioral and mental health, and social work standards for case management.
• Ability to coordinate admissions, transfers, transitions, and discharges with person-centered practices.
• Experience in interviewing, assessment, planning, intervention, and advocacy within health care settings.

Responsibilities

• Attends regular work on assigned shifts and supports ongoing care and treatment planning.
• Interviews individuals and representatives to assess support systems and needs for care planning.
• Oversees liaison and information exchange with treatment providers and families.
• Coordinates rights and capacity assessment and promotes informed decision-making.
• Participates in admissions, transfers, and discharge coordination with interdisciplinary teams.
