• Ability to restrain individuals as necessary to prevent injury and to work after hours, weekends, and holidays.

Responsibilities

• Performs professional social work services in treatment of individuals with intellectual disabilities on an assigned unit.
• Provides case management, including funding applications and liaison with community agencies.
• Develops discharge plans with the interdisciplinary team and families.
• Participates in daily/weekly team meetings.
• Prepares reports, forms, and correspondence accurately and timely.
• Assists families/guardians with understanding financial information and services provided.
