Job summary

Westside Federation of Senior & Supportive Housing is seeking a Social Worker I to provide autonomous, compassionate social work services to residents in the Bronx. You will build 1:1 relationships, coordinate access to medical and mental health services, and support independent living for residents.

The role offers a full-time schedule in Fischer Building, with opportunities for licensing support, supervision, and career development within a waivered agency.

Qualifications

• MSW or MA in human services field with a minimum of two years social work experience.
• Fluency in written and oral English required; bilingual a plus.
• Experience with older adults and people with mental illness and/or substance use preferred.

Responsibilities

• Assist residents with obtaining medical and mental health services, entitlements and home care.
• Establishing and/or maintaining family and social relationships.
• Budgeting, income maintenance etc. to ensure on-time payment of rent.
• Money management and crisis intervention services as needed.
• Individual counseling and in-house/community referrals for socialization activities.
• Oversee discharge planning and completion of forms and applications.
• Maintain thorough case records and participate in supervision meetings.
