Job summary

Westside Federation of Senior & Supportive Housing seeks a Social Worker I in The Bronx, NY. The full-time position involves developing 1:1 relationships with residents and providing a comprehensive range of social work services to promote safe, independent living.

The organization supports MSW licensing, offers study groups, covers licensing exams, CEUs, supervision, and a path to LCSW. Requires MSW/MA and English fluency; experience with older adults preferred.

Qualifications

• MSW or MA in human services field with a minimum of two years social work experience
• Fluency in written and oral English required; bilingual a plus
• Experience with older adults and people with mental illness and/or substance use preferred

Responsibilities

• Assist residents with obtaining and maintaining medical and mental health services, entitlements and home care
• Provide crisis intervention services, client appointment escorts, and individual counseling
• Assist residents in participating in socialization activities
• Follow up with residents and other service providers during hospitalizations or short-term rehabilitation
• Oversee discharge planning and complete necessary forms
• Provide group work services for residents and participate in intake processes
• Maintain thorough case records and confidentiality per policy
