Job summary

Jewish Family Service LA in North Hollywood, CA is seeking a Social Worker I to provide care coordination, housing-focused case management, and supportive services to older adults and dependent adults referred by APS.

You will perform intake, assessments, develop housing and service plans, conduct home visits, and coordinate benefits and resources to help clients maintain safety and housing stability.

Qualifications

• BA degree in Social Work or related field.
• Experience with case management and housing stabilization.
• Knowledge of community resources and public benefits.
• Strong verbal and written communication; telehealth familiarity.
• Experience with field visits and community-based service delivery preferred.
• Proficiency in MS Office and ability to learn new systems.

Responsibilities

• Provide care coordination and housing-focused case management to support safety and housing stability.
• Complete intake, enrollment, assessments, and obtain required client documentation.
• Develop individualized service and housing plans with goals related to stability, benefits, budgeting, and health.
• Conduct case management meetings in home, community, office, shelter, or other locations.
• Provide housing stabilization services including landlord mediation and eviction prevention.
• Assist clients with budgeting, government benefits, and financial planning.
• Coordinate relief financial assistance and ensure proper documentation and follow-up.
• Refer clients to County/community resources (DPSS, mental health, transportation, etc.).
• Maintain accurate documentation in required databases and uphold confidentiality and HIPAA.
