Job summary

Nathan Adelson Hospice Inc. seeks a per diem social worker to provide field-based psychosocial services in Nevada. You will perform comprehensive psychosocial assessments, identify patient needs, and offer supportive counseling, discharge planning, and resource assistance.

The role includes bereavement assessments for families, developing care plans with an interdisciplinary team, and active participation in team meetings to uphold the hospice mission.

Qualifications

• Bachelor's or Master's degree in Social Work approved by the CSWE.
• Minimum of one year supervised healthcare experience or during a supervised internship.

Responsibilities

• Conduct comprehensive psychosocial assessments.
• Identify problems and patient needs and provide solutions.
• Provide supportive counseling and discharge planning.
• Assist in accessing resources for patients and families.
• Conduct bereavement assessments for family members.
• Develop a plan of care with an interdisciplinary team.
• Participate in interdisciplinary team meetings.
• Support and promote the hospice’s vision, mission and philosophy.

Education

Job description

Social workers provide services that include a comprehensive psychosocial assessment, identification of problems and needs, supportive counseling, discharge planning, and assessment and facilitation relating to accessing resources. Additionally, they are responsible for a bereavement assessment of family members. Social workers are responsible for the development of a plan of care in conjunction with other interdisciplinary team members and regularly participate in the interdisciplinary team meetings. They actively support and promote the hospice’s vision, mission and overall philosophy.

Shift: Per Diem (on call / as needed) (in the field)

Education and Experience

• Bachelors or Master’s degree in Social Work approved by the Council on Social Work Education. A Master’s degree is preferred.
• Minimum of one (1) year supervised experience in the healthcare field or during a supervised internship.

Required Licensure and Certification

• A current license to practice as a Social Worker in the State of Nevada.
• Must be a licensed driver and possess an automobile that is insured in accordance with state and/or organizational requirements and is in good working order. Must maintain a clean driving record.
• Hospice and Palliative Care Certification is preferred.
