Job summary

Harbor Healthcare System in Clear Lake, TX is seeking a part-time Social Worker to provide social services to patients and families, coordinate treatment planning, and support discharge planning with the care team.

The ideal candidate holds a Licensed Social Worker license in Texas, has at least 1 year of licensed experience, and values clear written and verbal communication. Hospice or home health experience is preferred; a background check, drug screen, and driving record are required.

Responsibilities

• Provide social service activities related to patients and families, including treatment planning and consultation with staff.
• Assist in discharge planning by submitting written summaries of services and assessing patient status for charting.
• Participate in discharge planning and in-services programs as needed.

We are currently seeking qualified candidates for a part time Social Worker position in the Clear Lakearea.

Job Description:

Providing social service activity as it relates to the patients and their families. Thisincludestreatment, planning, and consultation with other staff members. Experiencein assessing the patient’s and their family’s emotional and social factors in orderto estimate their capacity and potential. Assisting in discharge planning from agency service when care is no longer appropriate by submitting a written summary of services provided. Thisincludesan assessment of the patient’s current statusto be retained in the patient’s chart. Participate in discharge planning, and in-services programs related to the needs of the patient.

Qualifications:

• LicensedSocial Workerin the State of Texas - required
• 1+ years'experience with license - required
