Job summary

Community Healthcare of Texas in Fort Worth provides hospice and palliative care since 1996. We are recruiting a Social Worker to provide therapeutic support, education, and resource information to patients and families.

You will advocate for patients’ needs with the care team, develop and execute the Plan of Care, document visits, and connect families with community resources. A Master’s in Social Work is required, along with reliable transportation and computer proficiency.

Qualifications

• Master's degree in Social Work required.
• Reliable transportation and a valid driver's license.
• Hospice or healthcare experience preferred.
• Proficient skills using a computer are required.

Responsibilities

• Assess the psychosocial needs of the patient and family members.
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues.
• Contribute to and execute the Plan of Care with the family/patient.
• Record daily documentation of visits and contact in accordance with Policy.
• Provides referrals to community resources.
• Performs other duties as assigned.

Community Healthcare of Texas is currently recruiting for a Social Worker.

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.

ESSENTIAL FUNCTIONS

• Assess the psychosocial needs of the patient and family members
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues
• Contribute to and execute the Plan of Care with the family/patient
• Record daily documentation of visits and contact in accordance with Policy
