Job summary

Harbor Healthcare System in the Lufkin area is seeking a full-time Social Worker to provide social services for patients and families, including discharge planning and coordination with care teams. The role requires licensure in Texas and strong communication skills.

Experience with Hospice or Home Health is preferred. A clean background, drug screen, and driving record are required as part of the onboarding process.

We are currently seeking qualified candidates for a full time Social Worker position in the Lufkinarea.

Job Description:

Providing social service activity as it relates to the patients and their families. Thisincludestreatment, planning, and consultation with other staff members. Experiencein assessing the patient’s and their family’s emotional and social factors in orderto estimate their capacity and potential. Assisting in discharge planning from agency service when care is no longer appropriate by submitting a written summary of services provided. Thisincludesan assessment of the patient’s current statusto be retained in the patient’s chart. Participate in discharge planning, and in-services programs related to the needs of the patient.

Qualifications:

• LicensedSocial Workerin the State of Texas - required
