Job summary

Pathways Hospice & Transition Home Healthcare is seeking a Social Worker (Hospice) PRN in the North Houston area to support patients, families, and the care team. The role focuses on improving or maintaining patients' social, emotional, and psychosocial well-being within a hospice setting.

Requires a Bachelor's or Master's in Social Work, Texas licensure, and at least two years supervisory experience in hospice or related medical home health.

Qualifications

• Bachelor's or Master's degree in Social Work; Texas licensure required.
• Minimum two years of supervisory social work experience in hospice or medical home health.
• Strong organizational and interpersonal skills; able to work with patients, families, and staff.
• Proficient with computer systems and EHR.
• In good standing with Federal and State Government.

Responsibilities

• Plans, develops, organizes, implements, evaluates, and direct the social services program of the hospice patient.
• Interviews resident and family during admission process to complete a social history and assessment of the patient to identify problem areas, and to develop rapport.
• Responsible for explaining and obtaining Advanced Directives with residents and families.
• Works with Nursing department to ensure accurate Do Not Resuscitate orders are obtained for patients.
• Develops and implements policies and procedures for identifying the medically-related social and emotional needs of the patient.
• With qualified supervision or with the appropriate license, applies Social Work Theory, knowledge, methods, ethics and the professional use of self to restore or enhance social, psychosocial, or biopsychosocial functioning of patients.
• Other duties as assigned

Social Worker (Hospice)

The Job:

Support the successful execution of our mission and Core Values by improving or maintaining each patients' ability to control everyday physical, mental, emotional, spiritual and psycho-social needs.

Responsibilities:

• Plans, develops, organizes implements, evaluates, and direct the social services program of the hospice patient

Must Haves:

• Must possess a Bachelors or Masters degree in Social Work, Must be a licensed social worker in the State of Texas.
• Minimum of two (2) years of social work experience in a supervisory capacity in a hospice or other related medical home health field
• Must have outstanding organizational skills andpossess the ability to deal tactfully with associates, patients, family members, visitors, government agencies/personnel and the general public.
• Proficient in the use of computer systems (Word, Excel, Electronic Health Record system, etc.)
• Must be in good standing with the Federal and State Government
