Job summary

Harbor Healthcare System in the Lufkin area is seeking a full-time Social Worker to provide social services for patients and families, including discharge planning and coordination with care teams. The role requires licensure in Texas and strong communication skills.

Experience with Hospice or Home Health is preferred. A clean background, drug screen, and driving record are required as part of the onboarding process.

Qualifications

• Licensed Social Worker in Texas (required).
• 1+ year experience with license (required).
• Hospice or Home Health experience preferred.
• Effective written and verbal communication.
• Clean background, drug screen, and driving record.

Responsibilities

• Provide social service activities related to patients and families.
• Assess emotional and social factors to estimate patient capacity and needs.
• Assist in discharge planning and submit written summaries of services provided.
• Participate in discharge planning and in-services programs related to patient needs.
