Job summary

Lighthouse Hospice NJ is seeking a licensed MSW (LMSW) to provide compassionate social work services to hospice patients and families in Mercer, Burlington, Camden, Gloucester & Salem counties. The role emphasizes counseling, emotional support, and coordination with the care team. Schedule is Monday to Friday 8 am–4:30 pm.

Must hold MSW from accredited school and NJ social work license, with a valid driver's license. Prior hospice or healthcare experience preferred.

Qualifications

• MSW from an accredited school of social work (required).
• Licensed as a Social Worker by the State Board of Social Work (required).
• Minimum 1 year hospice or healthcare setting experience preferred.

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and the interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement personalized social work plans and collaborate with the Bereavement Coordinator.

As a Licensed M edical Social Worker (MSW) with us, your primary responsibility will be delivering exceptional social work services to our hospice patients and their families. You'll provide counseling and guidance to support their emotional well-being and coping capacity. Your efforts will make a profound difference in their lives during challenging times.

• Be a Supportive Guide: Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.

• Provide Individualized Care Advocacy: Implement personalized social work plans and collaborate with your Bereavement Coordinator.

Qualifications

Education

• Required: Master of Social Work from an accredited school of social work

License Requirements

• Required: Licensed as a Social Worker from the State Board of Social Work
• Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle

Experience

• Preferred: Minimum one (1) year experience as a social worker in a hospice setting
• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

Additional State-Specific Requirements

AL, AR, GA, IL, MA, ME, NJ, NM, OK, OH, RI, SC, & TX: Current unencumbered license as a Social Worker from the State Board of Social Work
