Job summary

Crescent Hospice is seeking a Licensed Master Social Worker (LMSW) to provide compassionate social work services for hospice patients and their families in Florence and Sumter. You will assess psychosocial needs, advocate for care plans, and collaborate with an interdisciplinary team.

The role offers a Monday–Friday schedule (8:00am–4:30pm) and a $2,500 sign-on bonus, with a pay range of $30–$33 per hour. Comprehensive benefits and career growth opportunities accompany the position.

Qualifications

• Master of Social Work from an accredited school.
• Licensed as a Social Worker by the State Board.
• Current driver's license and access to a dependable vehicle.
• Minimum one year of hospice social work experience preferred; healthcare setting required.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager and interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement personalized social work plans and collaborate with the Bereavement Coordinator.
• Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
• Provide empathetic counseling to patients and families.

As a Licensed Medical Social Worker (MSW) with us, your primary responsibility will be delivering exceptional social work services to our hospice patients and their families. You'll provide counseling and guidance to support their emotional well-being and coping capacity. Your efforts will make a profound difference in their lives during challenging times.

Schedule: Monday - Friday, 8am - 4:30pm

Qualifications

Education

• Required: Master of Social Work from an accredited school of social work

License Requirements

• Required: Licensed as a Social Worker from the State Board of Social Work
• Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle

Experience

• Preferred: Minimum one (1) year experience as a social worker in a hospice setting
• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

Additional State-Specific Requirements:

SC: Current unencumberedlicense as a Social Worker from the State Board of Social Work

$30.00 - $33.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate's relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
