Job summary

Harbor Healthcare System in Independence, MO is seeking a full-time Social Worker to support patients and families, coordinate discharge planning, and collaborate with care teams to address emotional and social needs.

Candidates must be a Missouri Licensed Social Worker with at least one year of licensed experience; hospice or home health experience is preferred. Excellent written and verbal communication is required, along with a clean background, drug screen, and driving record.

We are currently seeking qualified candidates for a full time Social Worker position in the Independencearea.

Job Description

Providing social service activity as it relates to the patients and their families. Thisincludestreatment, planning, and consultation with other staff members. Experiencein assessing the patient’s and their family’s emotional and social factors in orderto estimate their capacity and potential. Assisting in discharge planning from agency service when care is no longer appropriate by submitting a written summary of services provided. Thisincludesan assessment of the patient’s current statusto be retained in the patient’s chart. Participate in discharge planning, and in-services programs related to the needs of the patient.

Qualifications
