Legacy Hospice is seeking a compassionate Social Worker to join our Independence, MO team. You will conduct psychosocial assessments, provide counseling, and assist with advance directives and navigating complex decisions for patients and families facing end-of-life care.

The role includes coordinating with community resources, educating staff on cultural needs, documenting interventions, and participating in team meetings to create individualized care plans.

Qualifications

• Bachelor’s degree in Social Work (BSW) required.
• Master’s degree (MSW) or LCSW strongly preferred.

Responsabilités

• Participate in quality improvement initiatives and continuing education.
• Participate in on-call support as needed.

Hospice Social Worker

Location: Independence, MO | Job Type: Hourly | Non-Exempt

In this vital role, you'll help patients and families cope with the emotional, social, and practical challenges that come with a terminal diagnosis—bringing comfort and clarity during their most vulnerable moments.

What You’ll Do

Qualifications

• At least 1 year of experience in social work, preferably in hospice, home health, or a community setting.
• Knowledge of end-of-life care, grief support, and cultural sensitivity.
• Strong written and verbal communication skills.
• Self-motivated, organized, and able to work independently.
• Current Basic Life Support (CPR) certification.
• Valid driver’s license, auto insurance, and reliable vehicle required.
