Hospice Social Worker
Location: Independence, MO | Job Type: Hourly | Non-Exempt
At Legacy Hospice, we’re committed to caring for patients and families with compassion and dignity. We are seeking a skilled and empathetic Social Worker (SW) to join our team. In this vital role, you'll help patients and families cope with the emotional, social, and practical challenges that come with a terminal diagnosis—bringing comfort and clarity during their most vulnerable moments.
What You’ll Do
- Conduct psychosocial assessments of patients and caregivers.
- Provide counseling to patients and families regarding emotional stress, coping, grief, and advance care planning.
- Assist with advance directives, funeral arrangements, and navigating legal/financial decisions.
- Identify and coordinate with community resources, agencies, and support organizations.
- Educate staff and families about cultural and psychosocial needs.
- Document patient conditions, interventions, and psychosocial support provided.
- Serve as a liaison between families and care teams to ensure coordinated, holistic care.
- Participate in interdisciplinary team meetings to develop individualized care plans.
- Provide crisis intervention and support for safety concerns (neglect, abuse, self-harm).
- Participate in quality improvement initiatives and continuing education.
- Participate in on-call support as needed.
Qualifications
- Bachelor’s degree in Social Work (BSW) required.
- Master’s degree (MSW) or LCSW strongly preferred.
- At least 1 year of experience in social work, preferably in hospice, home health, or a community setting.
- Knowledge of end-of-life care, grief support, and cultural sensitivity.
- Strong written and verbal communication skills.
- Self-motivated, organized, and able to work independently.
- Current Basic Life Support (CPR) certification.
- Valid driver’s license, auto insurance, and reliable vehicle required.
