Job summary

Trinity Health Senior Communities is seeking a qualified social worker to support residents and families in planning care, discharge planning, and assessments. You will coordinate with care teams and community resources to ensure smooth transitions and ongoing communication.

The role requires a social work degree, Michigan licensure, and experience in long-term care with seniors; a strong focus on customer service and adaptability is essential.

Qualifications

• One year of supervised social work experience in a healthcare setting with individuals.
• Experience in long-term care, working with the senior population and dementia counseling.
• Must be licensed as a social worker in Michigan.

• Flexibility to adapt to ongoing change and a fast-paced, customer-driven environment.

Job Responsibilities:

• Participate in planning related to resident, family, and community interests. Participate in discharge planning, development and implementation of social care plans and resident assessments. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, arranging transportation, etc., as necessary, coordinating activities with other departments as necessary.

• Promotes optimum transition of resident for appropriate and safe placement for discharge. Provides ongoing communication with resident and responsible persons of progress/status during stay.

• Provide information to the resident/family on Medicare/Medicaid as well as other financial assistance programs available. Provide consultation to staff, community agencies, etc., in efforts to solve resident concerns through the development of social service programs.

Minimum Qualifications:

• One year of supervised social work experience in a healthcare setting working directly with individuals.
