Job Overview

This role involves collaborating with multidisciplinary teams to assess, plan, implement, and evaluate psychosocial factors affecting patients' illnesses and continuity of care. Services may include discharge planning, counseling, crisis intervention, information and referral, and support for domestic violence, substance abuse, suspected abuse, psychiatric services, and guardianships. Services are provided both inpatient and outpatient.

Essential Responsibilities

• Complete and document psychosocial assessments, interventions, and care coordination for assigned populations.
• Provide crisis intervention and counseling to enhance patient and family coping mechanisms.
• Engage patients, families, and the interdisciplinary team in discharge planning and post-acute resource identification.
• Educate patients and families about psychosocial factors related to illness.
• Participate in activities that promote the role of social work within the hospital.
• Perform additional duties as assigned.

Qualifications

Minimum Requirements

• Master’s degree in Social Work (current employees may be exempted).
• Clearance under the Older Adults Protective Services Act (OAPSA), depending on location.
• Act 34 Criminal Background Clearance.
• Act 33 Child Abuse Clearance.
• Act 73 FBI Fingerprinting Criminal Background Clearance.

Preferred Qualifications

• LSW or LCSW eligibility preferred, depending on facility requirements.
• At least 2 years’ experience in a hospital or multidisciplinary healthcare setting.
