Job summary

Meriden Health & Rehab is seeking a Licensed Social Worker to join our care team in Meriden, CT. The role involves planning and delivering social services aligned with federal and state guidelines for skilled nursing residents.

You will work with families, residents, and the interdisciplinary team to develop and implement care plans, manage documentation, coordinate discharges, and participate in monthly QA and safety meetings to ensure optimal resident welfare.

Qualifications

• Experience as a LTC social worker within a nursing home.
• Knowledge of federal, state and local guidelines governing skilled nursing facilities.

Responsibilities

• Plan, organize, develop and implement social services to meet residents’ needs.
• Complete all social service documentation and admission assessments.
• Collaborate with the interdisciplinary team to create care plans for residents.
• Provide support to residents, families and staff; attend Care Plan and QA meetings.
• Assist with discharge planning and scheduling outside services.

• Performs all documentation requirements for social service documentation in a timely fashion.
• Completes all admission assessments and evaluations
• Works with the interdisciplinary team to create a plan of care for residents.
• Works with and provides support to residents, families and staff.
• Attends weekly Care Plan and Medicare meetings.
• Receives and attempts to resolve resident grievances.
• Coordinates resident discharge planning with all relevant departments and assists with scheduling outside services.
• Participates in monthly team meetings such as QA and Safety.
