Job summary

Meriden Health & Rehab in Meriden, CT is seeking a Part-time Social Worker (20 hours) to provide comprehensive social services to residents, in collaboration with the interdisciplinary team.

You will handle admissions assessments, documentation, discharge planning, and resident and family support, attending weekly care plan and QA meetings.

Qualifications

• CT Social Work License required.
• Experience as a social worker in a nursing home or rehabilitation center preferred.
• Admission experience: 1 year preferred.
• Long-term care experience: 2 years preferred.

Responsibilities

• Perform all social service documentation in a timely fashion.
• Complete all admission assessments and evaluations.

• Perform all documentation requirements for social service documentation in a timely fashion.
