Job summary

Meriden Health and Rehab LLC in Meriden, CT seeks a Licensed Social Worker to provide resident-centered social services within our skilled nursing facility.

The role requires a CT Social Work License and LTC experience; you will collaborate with an interdisciplinary team, support residents and families, and participate in care planning and QA activities.

The successful candidate will manage admissions, complete documentation promptly, and coordinate discharge planning with other departments.

Qualifications

• CT Social Work License is required.

Responsibilities

• Plan, organize, and implement social services for residents.
• Complete all social service documentation in a timely fashion.
• Complete admission assessments and evaluations.
• Collaborate with interdisciplinary team to create care plans.
• Support residents, families and staff.
• Attend weekly Care Plan and Medicare meetings.
• Receive and resolve resident grievances.
• Coordinate discharge planning with relevant departments.

Job Requirements

• Experience as a LTC social worker within a nursing home.
• Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities
• CT Social Work License (Required)
• Admission: 1 year (Preferred)
• Long term care: 2 years (Preferred)

Licensed Social Worker Responsibilities

• Plan, organize, development and implement all required social services to meet the needs of residents in accordance with federal, state and local guidelines that govern skilled nursing facilities.
• Performs all documentation requirements for social service documentation in a timely fashion.
• Completes all admission assessments and evaluations
• Works with the interdisciplinary team to create a plan of care for residents.
• Works with and provides support to residents, families and staff.
• Attends weekly Care Plan and Medicare meetings.
• Receives and attempts to resolve resident grievances.
• Coordinates resident discharge planning with all relevant departments and assists with scheduling outside services.
• Participates in monthly team meetings such as QA and Safety.
