Job summary

Meriden Health & Rehab seeks a Part-time Licensed Social Worker (20 hours) to support residents and families at our Meriden, CT facility. You will collaborate with an interdisciplinary team to enhance resident well-being and coordinate social services.

The role requires a CT Social Work License, experience in long-term care, and strong knowledge of regulations, with a commitment to compassionate, resident-centered care.

Qualifications

• Experience as a social worker within a nursing home or rehabilitation center is strongly preferred.
• Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
• CT Social Work License (Required)

Responsibilities

• Plan, organize, develop and implement all required social services to meet residents' needs per federal, state and local guidelines governing skilled nursing facilities.
• Complete all documentation requirements for social services in a timely fashion.
• Complete all admission assessments and evaluations.
• Collaborate with the interdisciplinary team to create a care plan for residents.
• Provide support to residents, families and staff.
• Attend weekly Care Plan and Medicare meetings.
• Receive and attempt to resolve resident grievances.
• Coordinate discharge planning with relevant departments and assist with scheduling outside services.
• Participate in monthly QA and Safety team meetings.
